Our Mission:

The mission of Disability Services at California Southern University (CalSouthern) is to promote access for all learners with disabilities who seek and use academic accommodations while ensuring equity, respecting diversity, and providing inclusion.

Our Purpose:

Disability Services provides academic accommodation(s) for all learners who are determined eligible based on disability documentation, the application process, and the learner’s self-report. Accommodations enable equitable access so that the academic progress of the learner with a disability in their major course of study is indicative of their abilities and not their disabilities.

Frequently Asked Questions (FAQs)

How to request academic accommodations for a disability.

  • Contact DisabilityServices@CalSouthern.edu to request the DDS Application.
  • The AIUS Request for Accommodations form must be completed by you and then submitted to a doctor, psychologist or clinician who can verify your disability.
  • If the disability is permanent, then you should ask the clinician to indicate that on the form.
  • The clinician needs to complete pages 2 and 3 of the form in its entirety, sign the form, date it, indicate their medical license number, and return it to you.
  • Once the AIUS Request for Accommodations form has been completed, you will need to scan it and email the form and/or documents back to Disability Services. Please email ALL documents to DisabilityServices@CalSouthern.edu

Types of Verification of Disability Documents  (list is not exhaustive)

  • Entire Psycho-Educational Testing Report (Required for Learning Disabilities, ADHD) – Tested as an adult (18 years and older). Excerpts of the report are not acceptable.
  • A Learner Disability Information and Verification Form – completed by a medical doctor or clinician who can attest to your disability.
  • Medical records identifying the name of the learner, diagnosis, and duration (Permanent or Temporary), doctor’s name and medical facility.
  • Letter from the doctor or clinician on office letterhead indicating the learner’s name, DOB, Diagnosis, Duration (Permanent/Temporary), signature, printed name and medical license number of the doctor or clinician.

Please be advised that Disability Services cannot accept Disability Verification documents that are:

  • Completed and/or sign by a doctor or clinician who is related to the learner by:
    • Genetics
    • Marriage
    • Co-habitation
    • Adoption
    • Domestic partnership
  • Completed based solely on asynchronous medical and/or psychological evaluations.

If I am Eligible for Services, What Happens Next?

  • The Coordinator of DSS will contact the learner by email to arrange a 15–20 minute New Learner Intake (NLI) appointment either by phone, Zoom or chat.
  • During the appointment, the Coordinator confers together
    with the learner in an interactive process to create an Academic Accommodation Plan and will explain the accommodations and how to request and use them.
  • Once the NLI is completed, the Coordinator will send emails through DocuSign consisting of two documents:
    • The Academic Accommodation Eligibility Agreement (stipulates the accommodations authorized by DSS including the duration of eligibility of services for the learner)
    • The Learner Rules and Responsibilities form (explains the policies of CalSouthern with respect to DSS)

Both documents should be signed online and returned to the DSS Coordinator. Once the documents are correctly completed and received by the Coordinator, the accommodation becomes available and may be used by the learner.

Important Note: Emails requesting disability accommodation(s) sent to mentors, Academic Advisors, Accounting, or other departments other than Disability Services may result in significant delays.
